3. PATIENT RESPONSIBILITY

• The practitioner is not responsible for third-party suppliers, manufacturers, or online retailers

6. INDEMNITY
To the fullest extent permitted by law, I agree to indemnify and hold harmless the practitioner from any claims, damages, or liabilities arising from:
• My use or misuse of recommended products
• Failure to follow advice or instructions
• Inaccurate, incomplete, or withheld information
7. NO GUARANTEES
I acknowledge that no guarantees or warranties have been made regarding outcomes.
8. EMERGENCY DISCLAIMER
I understand that this service is not intended for emergencies.
In the event of a medical emergency, I will seek immediate in-person medical care.

Some products offered by CeleRevive (including peptides and stem cell-based programmes) are not registered or approved by the South African Health Products Regulatory Authority (SAHPRA) or any equivalent regulatory body as medicines for the treatment, prevention, cure, or mitigation of any disease or medical condition.
These products are supplied strictly on a patient-specific basis only, following a consultation with and upon prescription by a licensed medical practitioner registered with the Health Professions Council of South Africa (HPCSA). They must be administered exclusively under the supervision and ongoing monitoring of a qualified healthcare professional.
